Proctored Exam Application

To All Online Students,

We recently had some technical problems with the Proctored Exam Application.  Anyone who completed an application on Tuesday, July 19th through Thursday, July 21st, we did not receive your request.  The problem has since been fixed, so we would like to ask you to please submit another application for your proctored exam at your earliest convenience.

New Proctored Exam Procedures

Dear Online Students:

This email is primarily for students who will need to make individual arrangements for a proctored exam using a testing service.  For the Spring 2011 semester, the SOM eLearning Office will launch a new way to complete a Proctored Exam Form.  In order to view the new procedures for arranging a proctored exam, please go to the Proctored Exam Information page:

 See procedure #3 to complete a Proctored Exam Application.  (For those of you who have already submitted a paper application, there is no need to complete the online application.)

Reminder: Scantrons!

If your course requires a proctored exam, and if your professor hasn’t already specified in the course syllabus, ask whether or not your proctored exam will require a Scantron. We (SOM eLearning) will not have that info until your prof sends us the exam, so the best place to find out is to go directly to the professor.

What is a Scantron, you ask? Below is a picture of the Scantron that professors have asked for most frequently. You can usually find them in at a university or community college bookstore. I’ve found them occasionally in grocery stores, but that’s hit or miss. My advice — don’t wait until the day before your exam to find one. Find them now, and maybe buy a pack of five or so (they’re not expensive at all) so that you’ll have them on hand for future proctored exams.

Proctored Exam Procedures

If you will be taking a proctored exam for one of your courses this semester, please fill out the proctored exam form (located on our information page: http://som.utdallas.edu/somResources/eLearning/eLearningCurrent/proctored.php). For most classes, the deadline has already passed; however, we are still accepting forms. If you have not turned your form in yet, please do so soon.

Once your proctored exam has been scheduled, please check with your testing center at least two days before your exam, so that we can troubleshoot if they have any trouble accessing the file.

Finally, please remember that proctored exams must be returned to UTD both by fax and mail. It is important that the exam is faxed, so that we have a backup in case the original is lost in the mail.

The deadline for returning the exams to UTD (the last day of the proctored exam window for your course) refers to the date by which we must receive the faxed copy — you do not need to express ship the original so that we receive it by that date.

Reminder: Contact your proctor ahead of time!

Hi guys. Just a friendly reminder to contact your proctors a couple of business days ahead of your scheduled exam time. Make sure they’ve received your exam and don’t have any questions. (If they do, let them know that they can contact us.) This way, there will be no problems when you show up for your exam on the testing date. We don’t mind taking care of the issue right then, but we know you all are already stressed out enough as it is, so let’s make this process as smooth as possible. Anyway, good luck with your midterms, finals, projects, and assignments!

Scantrons and Proctored Exams

Greetings folks! Now is a good time to check and see if your proctored final exam (if your course is actually having a proctored final) requires a Scantron. If so, the best place (that I’ve found) to successfully find these are at college/university bookstores. Sometimes you’ll find them at your local stores (i.e. Walmart, Albertsons, etc.) but it’s not consistent. Scantrons are also available online through the UTD Bookstore.

Proctored Exams – Get Started Early

Hi guys. Hope your semester is starting out well. About half of you are taking classes that require a proctored final exam. (Ethics students, you have both a proctored Midterm and Final.) I strongly recommend that you get this done early. Below are some useful links you should use in preparing for your proctored exam(s).

  • Proctored Exam General Information – Procedures and important links related to Proctored Exams.
  • Interactive Proctor Locations Map – Lists all testing locations used by our students to date. Keep in mind, you’re not limited to only using these locations. If you find a testing center not listed on this map, we’ll add it when we receive your proctored exam form.
  • Proctored Exam Form – This form is to be submitted by all students planning to take the exam outside of the scheduled in-class exam period listed in your syllabus. The student fills out and signs the top portion; the proctor fills out and signs the bottom portion.

Proctored Exams Reminder

Hi guys. Proctored exams are happening this week for several classes. Please remember to have your proctor fax a copy to me before mailing the original back to UTD. Once I get that faxed copy, the original can be mailed via normal mail — no need to use express couriers. The fax number is in the instructions that were sent out. Thanks!
